Narrative for MetroWest Medical Center - Leonard Morse Campus

Fall prevention plays a significant role in providing every patient with safe and quality care. Our interdisciplinary fall prevention program meets monthly in order to identify patient safety initiatives and discuss our progress towards zero falls. We have significantly decreased falls with injury in all patient care areas by remaining consistent when identifying high fall risk patients and initiating the appropriate interventions. We recognized a need for updated fall risk signage and safety alarms and have implemented the necessary changes. Ongoing education and research of evidence-based fall prevention practices will help us make future modifications so that we can be successful in preventing falls.

Our Pressure Injury consists of a collaborative multi-disciplinary program that meets monthly to identify opportunities for improvement and initiatives to progress to zero. We have decreased our pressure injuries and created initiatives for early identification. We are consistently reviewing evidence based practices and equipment to ensure every patient is receiving safe and quality care.
